League Event Registration

PARTICIPANT WAIVER AND RELEASE OF LIABILITY

This Participant Waiver and Release of Liability ("Agreement") is entered into voluntarily by the participant or the participant's parent/legal guardian (collectively, "Participant") in connection with participation in soccer league events, practices, matches, and related activities (collectively, "Activities") organized or facilitated by Gol Soccer Complex ("GSC"), located at 11501 Strathern St, Hollywood, CA 91605.

1. ASSUMPTION OF RISK

Participant acknowledges and understands that participation in soccer and related athletic activities involves inherent risks of injury, including but not limited to sprains, fractures, concussions, contusions, and other physical injuries, some of which may be serious or even fatal. Participant voluntarily assumes all risks associated with participation in the Activities.

2. WAIVER AND RELEASE

In consideration of being permitted to participate in the Activities, Participant, on behalf of themselves and their heirs, executors, administrators, and assigns, hereby releases, waives, and discharges GSC, its owners, officers, employees, volunteers, coaches, and agents (collectively, "Released Parties") from any and all liability, claims, demands, or causes of action arising out of or relating to any loss, damage, or injury, including death, that may be sustained by Participant, whether caused by the negligence of the Released Parties or otherwise.

3. INDEMNIFICATION

Participant agrees to indemnify and hold harmless the Released Parties from and against any claims, damages, losses, costs, and expenses (including reasonable attorneys' fees) arising out of or resulting from Participant's participation in the Activities.

4. MEDICAL AUTHORIZATION

In the event of an emergency, Participant authorizes GSC personnel to seek and obtain emergency medical treatment on Participant's behalf. Participant acknowledges that GSC is not responsible for any medical expenses incurred.

5. PHOTO/VIDEO RELEASE

Participant grants GSC the right to use photographs and videos taken during Activities for promotional and educational purposes without compensation.

6. RULES AND CONDUCT

Participant agrees to abide by all rules and regulations of GSC and to conduct themselves in a sportsmanlike manner. GSC reserves the right to remove any Participant for violations of conduct standards.

7. GOVERNING LAW

This Agreement shall be governed by the laws of the State of California. If any provision of this Agreement is found to be unenforceable, the remaining provisions shall remain in full force and effect.

8. ACKNOWLEDGMENT

By signing below, the Participant (or parent/guardian on behalf of the minor Participant) acknowledges that they have read this Agreement in its entirety, understand its terms, and voluntarily agree to be bound by it.
